For the first time ever, we are allotting space in our Junior Academy yearbook publication for 8th grade students to receive final messages from parents and/or relatives. The yearbook staff is now ready to take orders for these dedications. The dedications can include student name, up to 2 photos depending on size of space purchased, and a short message expressing love, congratulations, pride, etc., from their parents and/or family. Photo can be of student when they were younger and/or a more current picture.

GUIDELINES:

• Cash Only. No exceptions. Please place order form and payment in envelope.
• All orders must be returned directly to Ms. Palos in room 406. Do not submit orders to homebase teacher.
• There are two sizes you can choose from, but please note that there are a limited amount of spaces that will be sold of each size and all spaces will be sold on a FIRST-COME, FIRST-SERVED basis from May 6th to May 14th. 
• Only one dedication space can be purchased per student.
• You must FIRST submit this order form with your payment in order to reserve your spot.
• Photos and Messages: Photos must be submitted in a digital format (jpg). You may submit photo(s) on a CD, or email them to yearbook@sanjose-edison.com with the student’s name listed in the subject line. We cannot scan photos for you.
• Messages may be submitted in writing on the back of this form or you may email it to
  yearbook@sanjose-edison.com.
• Both photos and messages MUST be submitted no later than Monday May 17th. There will be
  NO EXCEPTIONS TO THIS DEADLINE!
• There will be no refunds made for spaces purchased with no accompanying photos and
  messages.

In the event that dedication spaces still remain after the deadline (May 14th), additional spaces will be made available to families, teachers, students, etc. who would like to purchase for other students, school groups, clubs, etc.
